1. Complete the "Authorization for the Scattering of Cremated Remains at Sea" form.

2. Attach a Bank Cashier's Check or money order for the appropriate amount made out to David Morin. A personal check will be accepted, but services will be delayed until the check clears. If you prefer to pay by credit card, please call (508) 278-0606.
    
3. Ship the cremated remains to:

   Uxbridge, MA 01569
   
   The U.S. Postal Service will ship ashes. Use Priority Mail with Certified Return Receipt Requested. Ashes must remain in the plastic/cardboard box in which you received them. Place the original box inside another box. Insert the completed Authorization for the Scattering of Cremated Remains at Sea and payment (if not paying by credit card) in the outer shipping container.
